Client Rights Notification

As a recipient of services at our facility, we would like to inform you of your rights as a client.

The information in this document explains your rights, and if you believe your rights have been violated.

Your Rights as a Client

1.   Grievances: We will thoroughly investigate any complaints you may have.

2.  Suggestions:  You are invited to suggest changes to any aspect of our services.

3.   Civil rights: Your civil liberties are protected by federal and state laws.

4.   Cultural, spiritual, or gender considerations: You may request services from someone trained or experienced in a specific cultural, spiritual, or gender orientation. If these services are unavailable, we will assist you with the referral process. 

5.   Treatment: You have the right to formulate your treatment plan.

6.   Denial of services: You may refuse the services offered to you and be informed of any potential consequences.

7.   Record restrictions: You may request restrictions on using or disclosing your protected health information; however, we are not required to agree.

8.   Request for Records: You can obtain a copy and/or inspect your protected health information; however, we reserve the right to deny access to certain records. If so, we will discuss this decision with you.

9.   Amendment of Records: You have the right to request an amendment to your records; however, this request may be denied. If denied, your request will be kept in the records.

10. Medical/Legal Advice: You may discuss your treatment with your doctor or attorney.

11. Disclosures: You have the right to receive an account of disclosures of your protected health information that you have not authorized.

Your Right to Receive Information

1.     Costs of services: We will notify you of the amount you will pay.

2.     Termination of Services: You will be informed about behaviors or violations that may result in the termination of services at our practice.

3.     Confidentiality: You will be informed about the limitations of confidentiality and how your protected health information will be utilized.

4.     Policy changes: You will be informed when a policy has been changed or updated. 

Our Ethical Obligations

1.     We are committed to serving the best interests of each client.

2.     We will not discriminate against clients or professionals based on age, race, creed, disability, sexual preference, or other personal characteristics.

3.     We have an objective and professional relationship with each client.

4.     We respect the rights and perspectives of other mental health professionals.

5.     We will terminate services or refer clients to other programs when necessary.

6.     We will continuously evaluate our personal limitations, strengths, and biases, as well as our effectiveness, to improve ourselves. We will continually pursue further education and training.

7.     We respect the various institutional and managerial policies and will assist in improving them if they serve the client's best interests.

Client’s Responsibilities

1.     As outlined in the Payment Contract for Services, you are responsible for your financial obligations to the clinic.

2.     You must adhere to the practice's policies outlined in the Financial Policy and Payment Contract for Services. 

3.     You are responsible for treating staff and fellow clients with respect and cordiality, so

         that their rights are not violated.

4.     You have a responsibility to provide accurate information about yourself. 


Client Data Collection, Use, and Protection

At Best Practice Psychotherapy, LLC, we are committed to safeguarding your personal information.
Below is an overview of how client data is collected, used, and protected:

Data Collection

1.       Personal Information: We collect personal information, including your name, contact details, demographic information, and health history, during the intake process and therapy sessions.

2.       Session Notes: Information from therapy sessions, including progress notes and treatment plans, is recorded to monitor your progress and customize your care.

3.       Surveys and Feedback: We may occasionally request feedback or conduct surveys to improve our services.

Data Use

1.       Treatment Planning: Your information is used to create personalized treatment plans that address your needs and goals.

2.       Billing and Insurance: Your data may be shared with insurance providers for billing purposes to ensure you receive coverage for the services rendered.

3.       Quality Improvement: We analyze client data anonymously to assess and improve our services and therapies.

1.       Confidentiality: We handle all client information with the utmost confidentiality and comply with HIPAA regulations to safeguard your health information.

2.       Secure Storage: Your data is stored securely in electronic format, and access is limited to authorized personnel only.

3.       Training: All staff receive training in confidentiality and data protection policies to ensure they understand the importance of safeguarding client information.

4.       Incident Protocol: In the unlikely event of a data breach, we have established strict protocols for addressing and notifying affected clients.
